Bedbug Annual Report Form (NYC Department of Housing Preservation and Development)
Requirement
- Owners of residential buildings with 3 or more units must annually collect information from tenants regarding bedbug infestations.
- A Bedbug Annual Report must be submitted to HPD, even if the building has no infestations.
Filing Process
- Hard copy forms are allowed only for individual or joint owners.
- All other owners must file online via HPD’s website.
- Buildings must be registered with HPD to file the report.
Reporting Period & Deadline
- Covers November of the previous year through October of the current year.
- Reports must be filed between December 1 and December 31.
- Failure to file by December 31 may result in violations.
Post-Filing Requirements
- Owners must post or distribute the filed report to tenants.
- Owners must also post or distribute the NYC Department of Health and Mental Hygiene (DOHMH) Bedbug Information Notice.
Form Sections
- Ownership Information: Identify the filer as an Individual Owner, Joint Owner, or Managing Agent.
- Infestation & Eradication Data:
- Total units in the building.
- Infested units during the reporting period.
- Units where bedbugs were eradicated.
- Units that were re-infested after treatment.
- Certification:
- Confirm whether the report will be posted in a public location or distributed to tenants upon lease renewal.
